UKS2 Only – My Health, My School Survey

We are writing to inform you about an opportunity for your child to take part in the My Health, My School survey, an annual survey commissioned by Leeds City Council and partners to gather valuable insights into the health and wellbeing of children and young people across Leeds.

The survey covers topics including physical activity, emotional wellbeing, diet, and healthy lifestyles. The information collected helps schools, local authorities, and health services to plan and improve services for young people.

Why We Need Your Consent:

In accordance with the UK General Data Protection Regulation (UK GDPR) and our Information Sharing Agreement (ISA) with Leeds City Council, we must obtain your explicit consent for the processing of your child’s personal information before they can participate in the survey. Pupils over 13 can give their own consent. Pupils will be given the choice to take part in the survey once we have received consent.

What You Need to Know:

· Participation is voluntary.

· The answers given will be anonymised and the survey does not collect names or any other personal data.

· Your child can choose to withdraw at any time, even during completion of the survey (at this point their data will be deleted).

· The survey is completed online during school hours and takes approximately 30 minutes.

· The completed survey data collected will be anonymised and used solely for research and planning purposes. No individual child will be identified in any published result.
